Wondering what the hell is going on, you roll for Arcane Knowledge and realize that she’s wearing a necklace with a Mind-Rape Gem on it; the gem will cause the wearer intense psychological and physical pain when a verbal command is given from the person who grew it, unless they have sex, in which case the pain is converted entirely into psychological torture, worsening if the sex is violent and decreasing if it is pleasant.
You know enough about the gem and its effects to remember only the giver of the gem can deactivate it, and right now that giver is Spike, who has been knocked out. You also do not know who did this to him, or why, and if he/she knew anything about the gem when it happened. In short, you do not know if the attacker is good or evil. All you know is that Rarity is at your mercy.
You have three options: work on removing the necklace through direct means, which you may be able to do with your knowledge, and help Rarity figure out just what is going on; contact Twilight Sparkle to lend her help, and have pleasant sex with Rarity to help her deal with the torture until Twilight arrives, fully aware it is still rape no matter what your intentions are; or violate her spectacularly until the gem overloads and turns her into a sexy vegetable. When you have made your decision, follow the consequences of the choice all the way through, deal with Spike, and find out who the mysterious assailant is.
