Oh yeah, it looks like the screenshot was taken seconds after the box was opened. Maybe the one who took it was waiting until the other two Derpies got close to the box…
I already know that, because her posture can tell that she’s the box Derpy.
The third Derpy is the one from the box. Ever since the game started, long before the other Derpies, there sometimes appears a box in the town. Clicking on it reveals Derpy underneath and she gives you bits and sometimes a gem (or more).
After that, as long as you keep her on screen she will hang out and interact with the other ponies (she will vanish the second you go off screen and return).
