I’m pretty sure Celestia is looking at Spike; not Fluttershy. Further, Applejack could just as easily be looking at either Spike or Fluttesrhy. Rarity is totally looking right at Spike’s bum, though.

I guess i find it odd that there’s an obvious focal point in this scene (severely injured Princess Celestia), yet the artist chose to have Rarity looking at Spike while Applejack looks at her with a look of disappointment. Maybe I could dismiss it if this wasn’t official comic material from an artist known for sneaking in random shoutouts.
