Twilight noticed that Spike is distracted in the first panel, which prompts her to noticed Glimmer is licking the tip, Ember got her tongue wrapped around, Trixie is taking it sideways with a little teeth, and Pinkie sucked hers dry. Twilight never knew there were other ways than just putting it in your mouth.
