I have a friend who has worked in proper (not fast-food) restaurants. He told me that spitting in the food does happen, but usually only if you’re really rude to the waitstaff. Somewhat ironically, it doesn’t happen in fast food places all that much for various reasons (like you point out, workers are monitored more closely for one).

As someone who’s worked at a Little Cezear’s, I’d say that is more of a myth. People working at fast food places are too closely watched and usually value their job too much to do something bad to your food. Of course, I’ve never worked at ALL fast food restaurants.
